CUKR card in the MOS system – the INTEURO Centre helps Ukrainian citizens prepare and submit their application
From 4 May 2026, applications for the CUKR residence card can be submitted through the MOS system – Moduł Obsługi Spraw.
The CUKR card is a document confirming the granting of a temporary residence permit under special conditions. It is issued for 3 years to Ukrainian citizens with PESEL UKR status who wish to move from temporary protection to a temporary residence permit in Poland. Applications for the CUKR card can be submitted from 4 May 2026 exclusively online through the MOS system – Moduł Obsługi Spraw.
At the INTEURO Centre, we can help you register in the MOS system and prepare and submit your application free of charge. However, it is very important that before your visit you prepare the necessary data and files in electronic form. This will make the whole process faster, easier and reduce the risk of mistakes.
📌 What should you prepare before your visit?
Before coming to the INTEURO Centre, prepare: a digital photograph, payment confirmations, details of a valid foreign passport, access to Profil Zaufany or another electronic signature, and check your PESEL UKR status.
1. Digital photograph
The photograph is one of the most important elements of the application. It cannot be an ordinary selfie. The photo should meet requirements similar to those for a residence card photo: a light, plain background, face looking straight ahead, current appearance and good file quality.
The file should be in JPG or JPEG format. The photo must be up to date, meaning it should have been taken no earlier than 6 months ago. If you only have a paper photo, it should be scanned in good quality. It is best to go to a photo studio and ask not only for printed photos, but also for an electronic version of the file.
Also pay attention to the technical parameters: the recommended photo size is 684 × 883 pixels, and the file size should not exceed 2.5 MB.
2. Payment confirmations
Before submitting the application, you need to prepare confirmations of two payments. It is best to make two separate bank transfers so that the documents are clear and easy to attach in the system.

The fee for issuing the residence card can also be paid in cash at the cash desk of the Świętokrzyski Urząd Wojewódzki in Kielce — building C, 5th floor, entrance from the Silnica river side. If you pay via online banking, download the payment confirmation in PDF format or prepare a good-quality scan.
3. Foreign passport details
When completing the application, you need to have the details of a valid foreign passport at hand: the series and number of the document, date of issue and expiry date. It is not always necessary to scan all passport pages immediately, but the data must be correct and up to date.
It is also important to check whether your current passport has been entered in the PESEL register. If you changed your passport after obtaining PESEL UKR but did not update your data at the municipal office, the MOS system may show an error or prevent you from completing the application correctly.
4. Access to Profil Zaufany or an electronic signature
The application for the CUKR card is submitted online and must be signed electronically. Therefore, you need an active Profil Zaufany, access to login.gov.pl, your login, password and a phone to receive an SMS code. An alternative may be a qualified electronic signature or a personal signature.
Without the possibility of signing the application electronically, it cannot be successfully sent. Also remember not to share your login details for Profil Zaufany, online banking or login.gov.pl with anyone.
5. Checking PESEL UKR status
Before starting to complete the application, it is worth checking whether you meet the basic conditions. A person may apply for the CUKR card if they had UKR status on 4 June 2025 and still have it, or if they have held UKR status continuously for at least 365 days.
If you have not previously submitted fingerprints, the system may accept the application, but later you may receive a summons to appear in person at the voivodeship office.
What should you do in the MOS system?
In the MOS system, you need to create a new user account. Even if you have used an older version of MOS before, the CUKR procedure may require creating or updating your profile in the new system. After logging in through login.gov.pl, you need to complete the electronic form, attach your photograph and payment confirmations, sign the application electronically and send it through the system.
After correctly signing and submitting the application, you will receive UPO — Urzędowe Poświadczenie Odbioru. This is the official confirmation that the application has been accepted in the system. It is worth saving it as a PDF file.
⚠️ Important
Submitting an application for the CUKR card through MOS does not require using paid intermediaries. Access to the MOS system is free of charge. Only the administrative fees are mandatory: PLN 340 and PLN 100.
